    Bids Are Invited For Custom Bid For Services - Supply Of Diet Items In Rlb Total Quantity : Additional conditions and restrictions of the tender invited on GeM portal for patient diet in Rani Lakshmi Bai Joint Hospital, Rajajipuram, Lucknow. Date - 12/09/23 Conditions and restrictions 1. The duration of the tender will be for one year from the date of contract or till the next tender and the estimated cost of which is approximately Rs 12.50 lakh. 2. It is necessary to attach the business tax registration certificate of the firm. And it is necessary to have a food related license granted by the competent authority, a copy of which should be attached. 3. It is mandatory to attach the photocopy of PAN card and Aadhaar of the bidder (firm/agency). 4. The tendering firm will have to give an affidavit on Rs 10 stamp that the concerned firm has never been blacklisted by any department or any government, the certificates being presented by the tendering firm. , they are true, and all the conditions and restrictions related to the tender are acceptable to them. 5. It is mandatory for the bidding firm to attach the character certificate issued by the District Officer. 6. It is mandatory for the tendering firm to have its local registered office in Lucknow. GST/MSME should also be from Lucknow. 7. It is mandatory for the tendering firm to have at least 3 financial years of experience in supplying food items in the Medical and Health Department
