The Subject Of The Contract Includes The Implementation Of Social Services For A Maximum Number Of 30 Elderly People Requiring Support In Their Daily Functioning At Their Place Of Residence, Which They Are 60 Years Old, In The Form Of Care Services. The Order Is Intended For Residents Of The Municipal Commune In Płońsk, Who Will Start The Project Increasing The Independence Of Seniors At Their Place Of Residence (Rmr) Implemented By The Municipal Social Welfare Center In Płońsk And Will Be Entitled To Use Social Services In The Form Of Care Services Based On The Declaration To Participate In The Project And Project Contract. Care Services Will Be Provided For The Maximum Number Of 30 People At The Place Of Residence The Residents Of The Płońsk Municipality- The Residents Of The Commune. Diseases. They Make In Particular: Care And Therapy Commissioned By A Doctor Who Includes Care Activities Resulting From The Submitted Medical Certificate Or Medical Documentation, Supplementing In Relation To Nursing Environmental Care; Satisfying Everyday Life Needs; Making Purchases, Dealing With Personal Matters; Sanitary; Help In Maintaining A Relationship With The Environment By Ensuring Contacts With The Environment; Improving To Function In Society; Maintaining Cleaning Rooms - Cleaning, Smoking In Furnaces, Washing Underwear; Customers Residence. The Number Of Hours Of Care Services Is Up To 15,360 (One Hour Of The Service Corresponds To 60 Minutes). Until The Service Is Provided, It Is Not Included In The Accession Time Or Reaching The Benefit Of The Apartment. The Number Of Services Of Services Has Been Given An Estimated One And May Change, Because It Cannot Be Determined Exactly Due To The Specificity Of The Order, Which Is Characterized By The Variability Of Needs Due To The Changing Health And Life Situation Of People Requiring Help. The Employer Specifies The Maximum Number Of Care Services In The Amount Of 15,360 Hours During The Performance Of The Contract. During The Performance Of The Contract, The Contractor Will Provide The Appropriate Number Of Persons Providing Social Services In The Form Of Care Services For The Correct Implementation Of The Task Covered By The Order. In Order To Perform Care Services Under This Order, The Contractor Is Obliged To Employ Persons Who Meet The Following Requirements: 1. They Have Appropriate Qualifications Or Experience In The Provision Of Care Services, In Particular In One Of The Following Professions Or Roles: • Environmental Guardian, • Assistant To A Disabled Person, • Nurse/Nurse, • Carer Of An Elderly Person, • Medical Guardian, • A Guardian Qualified At A Social Care Home, • A Person With Experience In The Implementation Of Care Services (Professional, Volunteer Or Personal), Resulting From The Performance Of Functions Actual Guardian. 2. They Completed Training In The Field Of Services Provided, In The Amount Of At Least 80 Hours, Also Covering The Subject Of First Aid Or Pre -Medical As
