Tenders Are Invited For Provision Of Neighborhood Assistance Services In The Mrocza Commune For Participants Of The Project Entitled The Functioning Of The Social Services Center In The Mrocza Commune For 2024-2026 - Ii Proceedings

The Subject Of The Contract Includes The Performance Of Services Provided In The Place Of Residence In The Mrocza Commune, Aimed At Enabling Dependent Persons To Function In The Current Environment, Despite The Agents In Independent Satisfaction Of The Basic And Necessary Needs And Barriers In Integration With The Environment, Taking Into Account The Following Terms Of The Contract: 1) Neighboring Services Include Help In Satisfying Basic Life Needs, Basic Care, Basic Care Hygienic -Publisory, Which Should Be Understood As Forms Of Support That Do Not Require Specialist Knowledge And Competence, And, In The Needs Of Needs And Possibilities, Providing Contacts With The Environment. 2) The Subject Of This Order Is The Provision Of Neighborhood Assistance Services Together For Min. 30 People For Each Year Of 2025 To 2027, I.E. From The Conclusion Of The Contract Until 30/09/2027), The Estimated Dimension Of The Number Of Hours Of Provision Of Neighborly Services During The Contract Is 14 400 Hours Support. The Expected Number Of Hours Of Neighborly Assistance Services Per 1 Participant In The Period Of Performance Is On Average 160 Hours Per Year. 5) In Connection With Ensuring The Security Of The Performance Of This Contract By The Contractor, The Contracting Authority Will Provide Data Of Persons Using Services In The Necessary Extent, In A Manner Specified In A Separate Agreement In The Case Of Processing Personal Data, Within The Meaning Of Art. 28 Para. 1 And 3 Of The Regulation Of The European Parliament Irada (Eu) 2016/679 Of 27 April 2016 On The Protection Of Individuals With Connection With The Processing Of Personally And On The Free Flow Of Such Data And Repealing Directive 95/46/Ec (General Regulation Of Data Protection; Journal Of Laws Of U. 2016, Item 119.1) 6) 4) And Points 5) The Number Of Hours Is An Estimated Amount And May Change (Increase Or Reduce To 30% Of The Number Of Hours), Because It Cannot Be Determined Exactly Due To The Order Of The Order, Which Is Characterized By The Variability Of Needs Due To The Changing Health Condition And The Life Situation Of People Who Dykes Help. The Employer Will Pay Only For Actually Completed Service Hours. The Value Of The Objecting Will Be The Product Provided By The Contractor Of The Unit Price Of The Service And The Number Of Hours Indicated Above. Contracts 9) The Contractor Providing The Neighborhood Assistance Service Declares Their Readiness To Implement/Provide 7 Days A Week, Beyond The Day, By Properly Determining With Persons Providing Hours Services And The Ordered Dimension And Scope Of Services. Service Services May Be Provided At Night (E.G. Sudden Illness Or Malaise) .10) The Contractor Is Obliged To Determine The Service Of Such A Number Of People Who Will Ensure An Efficient And Consistent Abuse Of The Ordering Party Of The Project Participants, Covered By This Order. 11) Tripartite Contract (Concluded Between The Participant, The Person Providing Services And The Entity
