A) The Subject Of The Contract Is The Provision Of Services Consisting In Providing Unlimited Access To Facilities And Sports And Recreational Activities As Part Of Personal Access Cards Made Available By The Contractor To Employees Of The Otwock City Hall And Subordinate Units Of Companies, Their Children, Accompanying Persons And Seniors. Sports And Recreational Facilities On The Same Day, Week Or Month Without The Need To Declare The Use Of A Specific Location And Time Of Day, During The Opening Hours Of Facilities And Conducting Sports And Recreational Activities, In A Way That Gives The Possibility Of Using The Services Of These Facilities Without A Time Limit. The Minimum Number Of Facilities In Poland Is 1000.D) The Offer Should Include A Package Of Various Sports And Recreational Services Provided By Sports And Recreational Facilities Within Which At Least Swimming Pools, Saunas, Gyms, Fitness Clubs, Aqua Aerobic, Yoga, Martial Arts, Indor Cycling, Spinning, Dance, Fitness With Dance Elements, Gymnastics Nordic Walking And Iinne.E) A Sports And Recreational Facility Means A Object That Provides Service Or Various Sports And Recreational Services At A Given Address Point. Various Sports And Recreational Activities Offered In One Facility At The Same Address By The Same Entity Will Be Treated As One Object.F) Access To The Provided Services Will Take Place On The Attitude Of Personal Sports Cards Issued By The Contractor For The Ordering Party, For Submitted Persons Authorized To Use Sports And Recreational Services Or In Other Ways Resulting From Technical And Organizational Solutions Used By A Given Pass Operator, E.G. G) Other Access Cards Covering Access To The Above -Mentioned Services Must Be Open, Which Must Be Understood By The Lack Of Any Restrictions In The Use Of Sports Facilities 7 Days A Week, With The Exception Of Restrictions Introduced By The Object Resulting From The Working Hours And Regulations In Force In It. In Order To Identify Persons, The Contracting Authority Does Not Provide For The Provision Of The Contractor More Information Than The Participants Name, And In The Case Of Children The Name, Surname And Date Of Birth. The Employer Allows The Transfer Of Other Data Necessary To Perform The Contract, Such As The Users Mobile Phone Number Directly By The User To The Contractor. The Contracting Authority Agrees That The Name Card May Be Verified With A Document Confirming The Identity And A Possible Signature On The List Of A Project Participant Or In Other Ways Resulting From Technical And Organizational Solutions Used By A Given Passes Of The Passes, E.G.
