1. The Subject Of The Contract Is The Delivery Of Laboratory Reagents For Hematological Tests Along With The Rental Lease For The Needs Of The Department Of Laboratory Diagnostics Of The Independent Public Health Care Team In Myszków. The Scope Of The Order Includes: A) Supplies Of Reagents And Consumable Materials For Hematological Tests - According To The List Constituting Annex 1.1 To Swz (Price Form), B) Rental Necessary To Perform The Abovementioned Research Of The System Components, Characterized In Annex 1.2 To Swz (Conditions For Lease Of The Analyzer) - In The Field Of Hematological Tests, While The Determination Of The System Is Understood: The Rented Device Together With The Right To Use The Software During The Contract During The Contract, Enabling The Contracting Authority To Perform Research Specified As To The Type And Quantity In Attachment 1.1 To Swzwrraz With The Analyzer And Humidity Meter To Monitor The Required Environmental Conditions. The Contractor Is Ensured That Thermometer And Humidity Meter Certification For A Period Of 3 Years. The Contractor And The Analyzer Will Also Provide A Microscope For Use In Hematology That Meets The Parameters Indicated In Annex 1.2.2. A Detailed Description And Conditions For The Implementation Of The Subject Of The Contract Are Determined By: - Price Form - Annex 1.1 To Swz, - Analyzer Rental Conditions - Annex 1.2 To Swz - Planned Contract Provisions - Annex 2 To Swz.
