1. The Subject Of The Contract Is The Service Of Collection, Transport And Disposal Of Hazardous Waste Of Medical Origin With Code 18 - All Subgroups And Types Of Waste. 2 Waste Collection Must Take Place At Least Three (3) Times A Week, I.E. Monday, Wednesday, Friday. 4. The Above -Mentioned Values Have Only An Estimated Nature And The Contractor May Not Claim Any Claims Related To The Transfer Of Smaller Amount Of Waste In The Indicated Periods. The Contracting Authority Reserves That The Minimum Order Level Will Be 70% Of The Value Of The Contract Constituting The Subject Of The Contract. 5. Quantitative Registration And Settlements Will Be Carried Out According To The Terms Of The Contracting Authority And Contractor. 6. Waste Transport Will Take Place By Means Of Transporting The Contractor Intended For Transporting Hazardous Waste In Accordance With Applicable Regulations. The Transport Of Waste Will Take Place In Accordance With The Requirements In The Field Of Environmental Protection As Well As The Safety Of Human Life And Health, In Particular In A Manner Taking Into Account The Chemical And Physical Properties Of Waste, Including The State Of Focus, And Threats That May Cause Waste, Including In Accordance With The Requirements Set Out In Separate Provisions. 7. The Contracting Authority Will Plan Each Time (Before Each Receipt) The Collection Of Waste In The Bdo System And The Contractor Will Confirm The Takeover Of Waste And Its Transport. The Basis For Issuing An Invoice Is Confirmation Of Issuing A Waste Transfer Card In The Bdo System. 8. Waste Disposal In Accordance With Art. 20 Para. 3 And Paragraph 6 Of The Act Of 14 December 2012 On Waste (I.E. Journal Of Laws Of 2022, Item 699) - The Principle Of Closeness Cited: The Disposal Of Infectious Medical Waste And Infectious Veterinary Waste - Outside The Voivodship In Which It Was Produced. Pursuant To Art. 20 Para. 6 Of The Aforementioned Act On Waste: Cutitization Of Infectious Medical Waste And Infectious Veterinary Waste In The Voivodship Other Than The One In Which It Was Generated In The Nearest Installation, In The Absence Of Installations For Disposing Of These Waste In A Given Voivodship Or When Existing Installations Do Not Have Free Excerper Capacity. The Terms Of The Contract Are Also Specified In The Contract Template (Part Ii Swz). 9. As Part Of The Contract, The Contractor Is Obliged To Provide Accompanying Services (E.G. Transport, Transport Insurance, Loading, Unloading, Etc.) Necessary For The Proper Performance Of The Contract.
