The Contractor Is Obliged To Perform Veterinary Services With The Preservation Of The Provisions Below: 1) Of The Act Of 21 August 1997 On The Protection Of Animals (Journal Of Laws Of 2023, Item 1580, As Amended), 2) Of The Act Of 11 March 2004 On The Protection Of Animal Health And Combating Infectious Animal Diseases (Journal Of Laws Of 2023, Item 1075, As Amended), 3) Of December 18, 2003 On Medicine Plant For Animal (Journal Of Laws Of 2019, Item 24), 4) Of The Act Of December 21, 1990 On The Profession Of Veterinarian And Medical And Veterinary Chambers (Journal Of Laws Of 2023, Item 154), 5) Of The Relevant Ordinances Issued On The Basis Of The Abovementioned Set. Requirements For The Contractor: 1) Must Be Entered In The Register Of Medical Plants For Animals Kept By The Relevant District Medical And Veterinary Council, 2) Has A Veterinary Office, Which Is Equipped With Appropriate Equipment For Performing Surgical Procedures, Specified In This Proceeding In The City Of Ełk, 3) Has Adequate Technical Facilities And Equipment In Offices Veterinary, Which Can Perform Veterinary Operations And Treatments, 4) Has Qualified Staff With Appropriate Permissions And Can Carry Out Veterinary Surgery And Treatments. The Services Of The Services Covered By The Orders: 1) Readiness To Provide Medical And Veterinary Services On Working Days On Business Days Between 17.00 - 8.00 And On Saturdays And On Saturdays And On Time, 2) Communication, In Which The Animal Is Injured Along With The Transport Of The Injured Animal To The Medicinal Institution And Taking The Animal Properly Care, 3) First Aid, General Examination Of The Injured Animal, Taking Actions Aimed At Stabilizing The Animals Health, 4) Performing Surgery With The Proviso That It May Be Performed Only If It Is Necessary To Save The Life, 5) In Order To Observe, Providing Them With Adequate Living Conditions, Meals And Water, 6) In Justified Cases, Euthanasia Treatment Together With The Transfer To Utilization. A Science Number Of Treatments From The Date Of Signing The Contract To December 31, 2025: 1) Readiness To Provide Medical And Veterinary Services On Business Days Between 17.00 - 8.00 And On Saturdays 250 Days, 2) Trips To Traffic Accidents, In Which The Animal Is Injured Along With The Transport Of The Injured Animal To The Medical Institution And Taking The Animal Properly Care - 3) First Aid, General Examination Of The Injured Animal, Undertaking Activities Aimed At Stabilizing The Animals Health - 38 Pcs, 4) Performing Surgery With The Reservation That It Can Be Performed Only In The Situation When It Is Necessary To Save Life-33 Pcs, 5) Ensuring A Place To Keep Animals For The Time Necessary To Perform The Services Referred To In P-Act 2, P-Dzień 3 And P-Pccie 4 Providing Them With Appropriate Living Conditions, Food And Water-44 (Day), 6) Euthanasia Of The Dog Together With The Transfer To Utilization-5 Pcs, 7) Euthanasia Of The Cat With Utilization-30 Pcs, 8) Euthanasia Of Other
