(HIRA) Health & Safety Representative Incident Investigation Basic Fire Fighting Please forward your (i)CIPC documents indicating share ownership or directorship of the company or a comprehensive CSD report which must indicate the names or the owner, their gender, race, age, whether there is a person living with disability(Medical certificate will be used to verify the disability states of the bidders) or not and the address of company preferred address on CSD in order to claim the preferential procurement points. (ii) Complete attached MBD Forms (iii) Municipal Account (for both company and directors) not older than 90days and written formal quotation provided on or before 14 April 2025 REQUIREMENTS: i. The total number of people to be trained for health and safety training is 16. ii. The trainer must be a South African Qualification Authority SAQA accredited provider with a valid unit standard for health & Safety alignment to OHSA. iii. The service provider should be an accredited and registered with Health and Welfare Sector Education and Training Authority (HWSETA). iv. The service provider must submit proof of accreditation number and valid certificate with Health And Welfare Sector Education and Training Authority or other relevant SETA v. The certificate of competence of the course must be provided within a month to delegates after attendance vi. The certificate must be valid for a period of at least two (2) years vii. Profile with contactable references not older than 3years must be submitted with the quotation/ proposal. viii. The venue will be provided by Musina Municipality within Musina Area. ix.
