The Subject Of The Contract Are Services Provided By Specialists In The Field Of Individual Consultations For Pupils Using The Short -Term Support Service In The Form Of A 24 -Hour Stay At The Municipal Social Services Center In Wrocław At The Nursing Home At Ul. Mączna 3 Or Ul. Karmelkowa 25 In Wrocław (Hereinafter Referred To As The Charges/ Project Participants). 2. The Subject Of The Contract Is Implemented As Part Of The Project Entitled Open House-Your Respite-Together In The Care Of Loved Ones No. Feds.07.07-Ip.02-0067/23-00 As Part Of Priority 7 European Funds For The Labor Market And Social Inclusion In Lower Silesia; Actions No. 7.7 Development Of Social And Health Services Of The European Funds Program For Lower Silesia 2021-2027 Co-Financed By The European Social Fund Plus. 3. The Subject Of The Contract Is Specified In The Code: Cpv 85121270-6 Psychiatric Or Psychological Services, 85300000-2 Social And Similar Work Services, Cpv 80340000-9 Special Education Services. 4. The Scope Of The Subject Of The Contract Includes, In Particular, Specialist Counseling Implemented By: A Psychologist Or Dietitian Or Speech Therapist, A Therapist Of Polyisoric Stimulation Or Other Specialists, E.G. A Typhlopedagogue Or A Special Educator Or A Music Therapist In An Amount Of 600 Hours During The Contract (The Contracting Authority Takes About 30 Hours A Month), Which Will Cover The Charges. Consultations Will Be Implemented As Part Of The Needs Reported By The Pupils. The Subject Of The Contract Includes In Particular: 1) Individual Consultations Of The Psychologist - Consultations Will Concern The Subject Of Integration And Activating In Particular: Building The Good Life Process Despite The State Of Health And The Situation Of The Project Participant, Motivation To Act And Improve The Quality Of Life, Supporting Preserved Abilities, Based On The Potential Of The Project Participant. The Forms Implemented Are Counseling, Activating Classes. Project Participants Will Be Able To Use Broadly Understood Psychological Services, I.E. Advice, Emotional Support, As Well As Classes Aimed At Slowing Down The Natural Processes Of Deterioration Of Cognitive Function, Or Classes Of Cognitive Function Stimulation And Psychological Functions; 2) Individual Consultations Dietitian - Consultations Will Concern The Subject: Knowledge In The Functioning Of The Body Under Sickness As Far As Possible And The Potential Of The Project Participant, Consultation Regarding Measurements Of Body Parameters And Its Balance; Working With People, E.G. With Depression Of Eating Problems Or Having Other Eating Disorders; 3) Individual Speech Therapist Consultations - Consultations Will Apply To The Subject: The Treatment Of Speech Disorders Addressed To Adults.
