1. The Subject Of The Contract Is The Design And Implementation Of Construction Works Along With The Delivery And Assembly Of Medical Equipment For The Provincial Hospital In Bielsko-Biała, Ul. Armii Krajowej 101, 43-316 Bielsko-Biała As Part Of The Task: Modernization Of The Central Operating Block Of The Provincial Hospital In Bielsko-Biała-Stage Iii. 2. The Subject Of The Contract Will Be Carried Out On The Basis Of: 1) The Functional And Utility Program (Pfu) Constituting Annex 3 To Swz, 2) Figure 1. Throw The Operating Theater Part. B Constituting Annex 3A To Pfu, 3) Figure No. 2. Projection Of Operating Room No. 5 Constituting Annex 3B To Pfu, 4) Figure 3. Creation Of A Robotic Operating Room With Facilities In The Central Operating Block Of The Provincial Hospital In Bielsko-Biała Annex 3C To Pfu, 5) Qualitative Parameters Of The Offered Equipment Constituting Annex 4 To Swz, 6) The Planned Provisions Of The Contract Constituting Annex 8 To Swz, 3. The Subject Of The Contract Will Take Place At The Contractors Cost And Risk. 4. Buildings Of The Provincial Hospital In Bielsko - Biała During All Construction Works Will Be Used. In Connection With The Above, The Contractor Is Obliged To Agree On The Date And Manner Of Performing The Most Burdensome Work With The Employer. 5. The Contractor Is Required To Read All The Factual And Legal Conditions That Are Necessary For The Proper Performance Of The Subject Of The Contract And Will Perform It Without Having To Bear Any Additional Costs By The Ordering Party. 6. Disconnecting, Omitting Elements Of Works Or Lack Of Recognition Of The Subject Of The Contract Cannot Be The Basis For Demanding The Change Of Price Specified By The Contractor. The Cost Of These Works Will Be Borne Only By The Contractor. 7. The Contractor Is Obliged To Make All Persons Performing Construction Works Necessary To Process The Contract, To Be Employed On The Basis Of An Employment Contract. The Requirement Of Employment Under An Employment Contract Does Not Apply To Persons Acting As A Construction Manager Indicated In The List Referred To In Chapter Ix Para. 1 Point 3 Swz.8. The Method Of Verification Of The Employment Of The Persons Referred To In Para. 7 And The Contracting Authoritys Rights Regarding The Control Of The Contractors Fulfillment Of The Requirements Related To Employing These Persons And Sanctions For Failure To Meet These Requirements, The Planned Provisions Of The Contract Constituting Annex 8 To Swz.9 Were Determined. The Contracting Authority Requires The Contractor To Conduct A Local Vision, From February 11, 2025 To February 12, 2025 From 12:00 To 14:00.10. The Contractor Is Obliged To Submit A Willingness To Make A Local Vision Via The Tender Platform Of The Provincial Hospital In Bielsko-Biała Https://Hospital.Logintrade.Net/Rejestraci/Stątcowe.Html At The Latest By February 7, 2025. The Application Must Contain The Name Of The Contractor. On The Basis Of The Application, The Contracting Authority
