1. The Subject Of The Contract Is The Contractor To Provide Nutrition Services To The Employer In Accordance With The Description Of The Service (Annex No. 1 To The Contract) And The Offer Form Attached To Swz (Annex No. 1 To Swz). 2. The Execution Of The Order Includes Daily Delivery Of Meals In The Diets Specified In The Description Of The Subject Of The Contract In An Estimated Amount Of 52436 Meals For A Period Of 12 Months (About 160 Hospital Patients A Day). 3. The Contracting Authority Absolutely Requires The Contractor To Dispose Of: 3.1. A Plant Preparing Meals (Kitchen) At A Distance From The Headquarters Of The Ordering Party Providing Delivery Of Hot Meals And 3.2. By A Car Transport, Adapted For Food Transport In The Catering System And Its Own Transport Trolley, Which Can Be Stored At The Contracting Authority, As Well As Washed And Disinfected By The Contractor At The Contracting Authority Or Taken By The Contractor As Well As Myths And Disinfected By The Contractor In Own Rooms Of The Contractor. The Plant (Kitchen) And The Means Of Transport Contractors Must Meet All Sanitary Requirements Specified By The Relevant Provisions And Recommendations, Guidelines Of Control Bodies For Food Safety And Nutrition. Contractors Working In Contact With Food Should Have A Current Booklet For Sanitary And Epidemiological Purposes, I.E. Have A Current Medical Certificate For Sanitary And Epidemiological Purposes About The Lack Of Contraindications To Perform Work, When It Is Possible To Transfer Infection To Other Persons In Accordance With Art. 59 Of The Act Of 25 August 2006 On Food Safety And Nutrition (I.E. Journal Of Laws Of 2023, Item 1448). The Contracting Authority Has The Right To Check The Validity Of The Current Medical Certificate. Employees Of Contractors Transporting Meals And/Or Poconsum Waste Should Have A Car Card For Inspection For The Contracting Authority With The Provisions Of The Car Washing And Disinfection Process As Well As Washing And Disinfection Of The Transport Trolley. Employees Of Contractors Transporting Meals And /Or Inproductive Waste Should Be Neatly Dressed, Neat, Cleaned, Must Be Wearing A Clean Apron And Disposable Gloves That The Contractor Provides On Their Own. The Contracting Authority Has The Right To Control. 4. A Detailed Description Of The Method Of Implementing The Abovementioned Service Orders Are A Template Of The Contract - Annex 6 To Swz. The Contractors Duty Is To Read The Swz Content, Its Attachments And Obtain All Information Necessary For The Proper Preparation Of The Offer. 5. The Contracting Authority Informs That He Is Participating In The Good Meal Program, Therefore The Selected Contractor Will Provide Meals In Accordance With The Regulation Of The Minister Of Health Of September 25, 2023. On The Pilot Program In The Field Of Nutritional Education And Improving The Quality Of Nutrition In Hospitals - Good Meal In The Hospital.
