The Subject Of The Contract Is The Performance Of Diagnostic Laboratory Tests Listed In Detail In Price Forms - Annex 2A - Nfz Research And Annex 2B - Tests Payable To Swz.A) Tests Materials Will Be Taken By An Employee Of The Municipal Health Center In Wodzisławiub) Employer Laboratory Every Day From Monday To Friday From 12:00 To 14:00. C) The Contractor Provides All Equipment Needed To Download The Material For Testing. The Contractor Protects The Transport Of Material For The Tests And All Accessories Necessary For Proper Transport And Storage Of The Material - From The Moment Of Receipt Of The Material To The Tests From The Headquarters Awarding The Order Until The Tests Were Completed In The Contractor Laboratory. Research.E) The Research Will Be Completed By A Written Result Signed By A Person With Appropriate Qualifications And Permissions To Approve The Research In Question. The Test Result Will Also Be Sent To The Employer In The Form Of On - Line In Real Time (Access To Test Results At The Subcontractor). In Addition To The Electronic Version Of The Test Results, The Contractor Will Provide The Employer With Paper Results In Paper Form Every Day From Monday To Friday During The Laboratorys Working Hours. G) The Award Of The Contract Requires The Contractor To Secure The Continuity Of The Provision Of Services, And In The Event Of Circumstances Preventing The Performance Of Services That Are The Subject Of The Contract Lasting Over 2 Days, A Written Indication By The Contractor By Another Place Of Their Implementation Under The Terms Specified In The Terms Specified In The Contract. H) The Tests Will Be Carried Out Using Cameras Or Devices Meeting The Standards Specified By The National Health Fund.I) The Contractor Will Provide The Employer For The Duration Of The Contract Within Two Weeks Of Conclusion Of The Contract, Appropriate Computer Equipment For Correctly Recording And Archiving Health Services With The Possibility Of Registering And Receiving The Results Of The Research In Electronic Version (Online). J) The Contractor Undertakes To Integrate The It System With The Ordering Party System Without Charging Additional Costs. The Contracting Authority Works On The M Medica System Used By The Contractor, The System Should Be Compatible With W.2. The Contractor Will Ensure The Performance Of Research In Accordance With Applicable Standards, Taking Into Account Quality Requirements And System Procedures, Among Others In Accordance With The Requirements Set Out In The Provisions: 1) Of The Act Of 15 September 2022 On Laboratory Medicine (Journal Of Laws Of 2023, Item 2125 As Amended); 2) Regulation Of The Minister Of Health Of 16 July 2024 On The Requirements To Be Met By The Medical Diagnostic Laboratory And The Qualifications Of Staff (Journal Of Laws Of 2024, Item 1188 As Amended);
