I. The Subject Of The Contract Is The Provision Of Transport Services For The Polkowice Health Services Center - Zoz S.A., In Mainly The Lower Silesian Voivodship, In Particular The Polkowice, Lubin, Głogów Commune. Detailed Description And Manner Of The Order: In The Scope Of Providing Transport Services 1) Services Will Be Provided From Monday To Sunday Between 7.00 - 20.00, 365 Days A Year; 2) The Contractor Has Min. 1 Passenger Car (5 People) And Available At The Request Of 1 Bus Passenger Car (Adapted To The Transport Of Furniture, Large Cardboard Boxes, 2, 5, 10 Liters, Medical Equipment, Etc.). Cars Should Be Kept In Impeccable Cleanliness. 3) Waiting Time For Passenger Transport Not Longer Than 15 Minutes. 4) Services Will Be Provided On The Principles Of Non -Cash Settlements On The Basis Of Completed Transport Cards, On Which The Employers Employee Will Confirm The Performance Of The Transport Service On An Ongoing Basis. The Contractor Provides Payment For The Performance Of The Service Based On The List Of Transferred Transports Performed In The Month Preceding The Issue Of The Invoice With Orders/Transport Cards Constituting The Basis For The Settlement. The Contractor Shall Transfer The List Of Transport To The Ordering Party In Writing. 5) The Contracting Authority Requires That The Contractor Has Valid Insurance Contracts, Including Civil Liability Insurance, Ac, Nnw And Valid Technical Examinations Of The Vehicle Throughout The Entire Performance Of The Contract. In A Situation Where The Insurance Contract Expires During The Contract, The Contractor Will Be Obliged To Conclude A New Insurance Contract At Such A Time Limit As To Ensure The Continuity Of The Performance Of The Contract. The Value And Material Scope Of The Insurance Contract Must Be In Line With The Legal Provisions In Force In This Respect. 6) In The Period From April To September, Cars Equipped With Air Conditioning. 7) The Contractor Will Provide A Driver On His Own. Topography Of The Lower Silesian Voivodship.
