Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including Form CAC2 and CAC7 (or CAC1.); b. Evidence of Companys Income Tax Clearance Certificate for the last three (3) years valid till 31st December, 2024; c. Evidence of current Pension Compliance Certificate valid till 31st December; d. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2024; e. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Certificate valid till 31st December, 2024; f. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2024 or valid Certificate issued by BPP. g. Sworn Affidavit disclosing whether or not any officer of the relevant committees of Federal Medical Centre Birnin Kudu or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidd4er and to confirm that all information presented in its bid are true and correct in all particulars; h. Companys Audited Accounts for the last three (3) years -2021, 2022 & 2023; i. Evidence of financial capability to execute the project by submission of Reference Letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed; j. Verifiable documentary evidence of at least three (3) similar jobs (in size, nature & complexity) executed in the last five (5) years including Letter of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects; k. Evidence of dealership or manufacturers authorization in accordance with the approved policy. (For all the lots, A, B & C). l. All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firms Letter Head paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.) and e-mail address. The Letterhead paper must bear the Name and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm. Bid documents shall be obtained at the Procurement Department on payment of Ten Thousand naira (N10,000.00) only per Lot to Federal Medical Centre Jigawa, at any Commercial Bank through the Treasury single account platform or at the centres cash office within the Hospital premises. Completed Technical and Financial bids in separate envelopes shall be enclosed in a large envelope labeled technical and Financial bids with the project Name at the top right hand corner and the company Name behind and shall be addressed to the Medical Director Federal Medical Centre Birnin Kudu, Jigawa State, PMB 1022. The document should be deposited in the Tender Box at the Head of Procurement office on or before 12:00 noon on Tuesday 11th February, 2025. Completed Technical Bids shall be opened immediately after deadline for submission at 12:00 noon on Tuesday 11th February, 2025, at the Hospitals conference Hall, while financial bids will be kept un-opened 6. GENERAL INFORMATION a. Bids must be English Language and signed by an official authorized by the bidder; b. Bids submitted after the deadline for submission would be returned un-opened; c. Proof of payment on Tender fee must be attached. d. Standard Bidding Documents can also be collected in Soft copies and Bidders can submit through Post (Courier Services). e. Presence of Bidders/representative from civil societies organization and anti-corruption transparency members is required to witness the exercise. f. Bidders cannot bid for more than one lot at a time. g. The FMC Birnin Kudu is not bound to pre-qualify any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act, 2007.
