Conditions Required From Contractors - Contractors Submitting An Offer In The Procedure Declare That They Undertake To Comply With The Following Requirements: 1) They Will Offer Medicinal Products That Meet The Requirements Specified In The Act Of September 6, 2001 Pharmaceutical Law (Journal Of Laws 2024.686, I.E. Of 2024/05/06, As Amended), In Accordance With The Description Of The Subject Of The Order Contained In The Forms Assortment And Prices Constituting Annex No. 3 To The Swz. (Packages No. 7 And 10)2) The Contractor Has Documents Confirming That The Products Listed In Annex No. 3 To The Swz Are Registered With The Office For Registration Of Medicinal Products, Medical Devices And Biocidal Products In Accordance With The Requirements Of The Act Of September 6, 2001. Pharmaceutical Law (Journal Of Laws 2024.686, Consolidated Text Of 2024/05/06, As Amended).3) For Medicinal Products In Packages (Packages No. 7 And 10), The Ordering Party Requires A Valid Gif Permit To Trade In Medicinal Products And A Gif Permit To Trade Narcotic Drugs And Psychotropic Substances.4) The Ordering Party Requires That Medical Devices Have A Udi Code Carrier On The Label And At All Higher Levels Of Packaging In The Case Of Class Iii Devices, Implantable Devices And Class Iia And Iib Devices, If This Applies To The Selected Medical Device.5) The Contractor Will Ensure, At Its Own Expense And Risk, The Transport Of Medicinal Products To The Place Indicated By The Ordering Party. Transport Of All Medicinal Products Will Be Carried Out In Accordance With The Requirements Provided For In The Current Documents Confirming The Authorization Of The Subject Of The Order For Marketing And Use In Accordance With The Act Of September 6, 2001 Pharmaceutical Law (Journal Of Laws 2024.686, Consolidated Text Of May 2024, 2024, As Amended). (Packages 7 And 10) The Ordering Party Has The Right To Inspect The Means Of Transport In Which Medicinal Products Are Transported For The Presence Of Records Of The Temperature In The Transport Chamber And The Presence Of Platforms On Which Boxes Of Medicinal Products Are Transported. In The Event Of Failure To Meet The Transport Requirements Provided For In The Current Documents Confirming The Admission Of The Subject Of The Order To Trading Provided For In The Act Of September 6, 2001, Pharmaceutical Law (Journal Of Laws 2024.686, Consolidated Text Of May 2024, As Amended), The Ordering Party Has The Right To Refuse Them.
