The Subject Of The Order Is The Provision Of Speech Therapy Services For The Needs Of The Integration And Therapeutic Center For Children And Youth In Brześć Kujawski - Group 2. The Order Concerns The Provision Of Ongoing Activities Of The Integration And Therapeutic Center For Children And Youth (Day Support Facility). The Subject Of The Order Is The Provision Of Speech Therapy Services, Which Are Addressed To People At Risk Of Poverty Or Social Exclusion, Including Students/Children Of Preschool Age Who Have A Certificate On The Need For Special Education Issued Due To A Given Type Of Disability, And Children And Adolescents Who Have A Certificate On The Need For Rehabilitation And Educational Classes Issued Due To Profound Intellectual Disability (These Certificates Are Issued By An Adjudicating Team Operating In A Public Psychological And Pedagogical Counseling Center, Including A Specialist Counseling Center) By Persons Qualified To Practice The Profession, I.E. Speech Therapist, Performed At The Integration And Therapeutic Center For Children And Youth, Ul. Królewska 5, 87-880 Brześć Kujawski From January 2, 2025 To December 31, 2025 (Excluding The Summer Break Lasting From July 15, 2025 To August 15, 2025)1) Speech Therapist Service - Common Room Classes Will Be Conducted In The Form Of An Individual Speech Therapist Service In Total: 440 Hours. Speech Therapists Duties In Addition To Providing Services The Above-Mentioned Services Will Include Preparing Teaching Materials, Schedules Of Classes, Maintaining Settlement Documentation (I.E. Attendance Lists, Course Sheets, Class Diary, Monthly Worksheet Of The Contractor - Protocol) And Informing Him About Cases Of Participants Absence From Classes. Place Of Provision Of Services .The Place Of Providing Speech Therapy Services For The Integration And Therapeutic Center For Children And Youth In Brześć Kujawski Is The Building Brzeski Social Welfare Center In Brześć Kujawski (Adapted To The Needs Of People With Disabilities) At 5 Królewska Street In Brześć Kujawski - Integration And Therapeutic Center For Children And Youth (Day Support Facility). Schedule Of Classes For The Therapist: A) Speech Therapy Service - 440 Hours During The Period: January 2, 2025 - December 31 2025 B) Speech Therapy Classes Will Not Take Place During The Summer Break In The Integration And Therapeutic Center For Children And Youth In The Period From July 15, 2025 To August 15, 2025. C) The Above-Mentioned Services Will Be Conducted In Accordance With The Ordering Partys Arrangements Within The Hours From 07.00 To 18.00, 6 Days A Week, From Monday To Saturdays (Excluding Public Holidays. On Public Holidays, Classes Are Not Held And Are Not Subject To Make-Up). D) Detailed Meeting Schedule Plans Will Be Agreed With The Contractor After Signing The Contract(S). E) The Ordering Party Will Pay Remuneration For The Service Provided From Its Own Funds.
