1.1. The Subject Of The Order Is The Provision Of Personal Assistance Services Referred To In The Personal Assistant For Persons With Disabilities Program - 2025 Edition, Hereinafter Referred To As The Service To The Residents Of The Międzyrzecz Commune Who Are Participants Of The Program. Services Provided In The City And Commune Of Międzyrzecz, I.E. In The Place Of Residence Of The Resident Covered By The Services And In His Immediate Surroundings. 1.2. The Personal Assistance Service Will Be Provided To Persons Qualified For The Program Who Meet The Criteria Specified In The Program1) Personal Assistant For Persons With Disabilities Program For Local Government Units - 2025 Edition Https://Www.Gov.Pl/Web/Uw-Opolski/Asystent-Osobisty-Osoby-Z-Niepelnosprawnoscia-Dla- Jednostek-Samorzadu-Terytorialnego---Edycja-20251.3. In The Case Of Aoon Program Participants, Services Can Be Provided 24 Hours A Day, 7 Days A Week, By The Same Assistant For A Maximum Of 12 Hours A Day. 1.4. The Main Goal Of The Personal Assistant For Persons With Disabilities Program - 2025 Edition Is To Support Disabled People, Including: 1) Children From The Age Of 2 To The Age Of 16 With A Disability Certificate, Including Indications Of: The Need For Permanent Or Long-Term Care Or Assistance Another Person Due To The Significantly Limited Ability To Live Independently And The Need For Constant Daily Participation Of The Childs Guardian In The Process Of Treatment, Rehabilitation And Education, And 2) Adults Disabled People With A Certificate Of: A) Significant Degree Of Disability Or B) Moderate Degree Of Disability Or C) Treated On An Equal Footing With The Certificates Referred To In Point (A). A And B, In Accordance With Art. 5 And Art. 62 Of The Act Of 27 August 1997 On Vocational And Social Rehabilitation And Employment Of Disabled People (Journal Of Laws Of 2021, Item 573, As Amended). 1.5. Personal Assistance Services Include Care For Disabled People, Mainly Those Who Are Lonely, In Performing Everyday Activities And Functioning In Social Life. 1.6. The Contractor Is Not Entitled To Reimbursement Of Travel Costs To And From The Place Of Service, Purchase Of Meals, As Well As Other Costs Incurred During The Provision Of The Service.1.7. The Contractor Will Keep Records Of The Purchase: A. Public/Private Transport Tickets For The Assistant Accompanying The Participant, B. Entry Tickets To Cultural Institutions, Cultural, Entertainment, Sports Or Social Events, Etc. By An Assistant Accompanying The Participant.1.8. Personal Assistance Services As Part Of A Day Stay Will Be Provided In The Place Of Residence Of The Disabled Person.1.9. The Number Of Hours Of Support Provided To The Assistant And Caregiver May Not Exceed The Limits Specified In The Personal Assistant For Persons With Disabilities Program - 2025.2 Edition.
