The Subject Of The Order Is The Provision Of Services In The Field Of Collection, Transport And Storage Of Human Corpses In A Cold Store And Autopsy Of Human Corpses For Sp Zoz Mswia In Kielce. Saint John Paul Ii. The Order Is Not Divided Into Parts. The Service Consists Of Two Tasks: Task 1: Collection, Transport And Storage For Up To 72 Hours Of The Bodies Of Patients Who Died From Sp Zoz Mswia In Kielce. Saint Jana Pawła Ii, Ul. Wojska Polskiego 51 In Kielce To A Cold Store Located Within A Driving Distance Of Up To 7 Km. The Body Is Transported To The Cold Store No Earlier Than Two Hours After The Time Of Death Indicated On The Death Certificate. Corpses Are Collected From The Hospital 24 Hours A Day, Every Day Of The Week, Within 3 Hours Of The Telephone Notification By The Hospital Staff. Task 2: Performing Autopsies Of Deceased Patients At The Sp Zoz Mswia Hospital In Kielce. Saint Jana Pawła Ii, Ul. Wojska Polskiego 51 In Kielce. The Contractor In The Offer Form - Annex No. 2 Of The Swz Provides The Gross Offer Price Calculated On The Basis Of The Estimated Number Of Services Specified By The Ordering Party For The Duration Of The Contract. The Ordering Party Determined The Expected Number Of Services Over A 12-Month Period To Be Approximately 350 Deaths. (I.E. 24 Months/Approx. 700 Deaths For The Entire Duration Of The Contract Concluded In These Proceedings). A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1 - Constituting An Integral Part Of The Contract.
