Tenders Are Invited For Providing Services Of Collection, Transport And Storage Of Bodies Of Deceased Persons At The Hospital In Ostróda S.A. - Procedure No. 2

1. The Subject Of The Order Includes: 1) Collection Of Bodies Of Deceased Persons From Hospital Wards Or Another Place Indicated By The Ordering Party After 2 Hours From The Time Of Death. The Basis For The Provision Of The Service Is An Instruction Given By Telephone, Fax Or Electronically By Authorized Employees Of The Ordering Party (Nurse, Ward Nurse, Midwife). The Contractor Must Provide, At His Own Expense And On His Own, A Trolley And Bags Suitable For Transporting Bodies. The Body Is Released On The Basis Of The Completed Corporate Referral Card To The Mortuary/Cold Storage - According To The Template Attached As Annex No. 1 To The Agreement.2) Collection, Transport, Burial Or Cremation And Burial Of Remains Consisting Of Ashes Resulting From The Cremation Of The Bodies Of Stillborn Children Regardless Of The Duration Of Pregnancy And Not Collected By The Parents - Performed In Accordance With The Provisions Of The Regulation Of The Minister Of Health Of December 7, 2001 On The Treatment Of Human Corpses And Remains (Journal Of Laws Of 2021, Item 1910).3) Providing The Ordering Party With An Opportunity To Conduct An Autopsy In Rooms That Meet The Requirements Specified In The Regulation Of The Minister Of Health And Social Welfare Of April 10, 1972 On Occupational Health And Safety In Pathological Anatomy Departments, Dissection Rooms And Histopathological And Histochemical Laboratories (Journal Of Laws No. 17, Item 123). 4) Providing Assistance Of An Autopsy Technician During Autopsy At The Request Of The Ordering Party. The Autopsy Will Be Carried Out On The Basis Of The Section Card And An Order In The Form Of A Referral Card For Autopsy And Documentation Circulation - Prepared According To The Templates Constituting Annexes No. 2 And 2A To The Contract, By A Doctor Indicated By The Ordering Party And Having The Required Qualifications In This Respect (Within 12 Hours Of Confirming Death And Within 60 Hours Of Confirming Death) And Documenting Them In Accordance With Applicable Regulations.5) Transporting The Body With The Contractors Transport To The Mortuary/Cold Storage Room.6) The Ordering Party Is Obliged To Deliver It To The Contractor Based On The Referral Card, The Body Of The Deceased Person Should Be Clean, Tidy, Secured (Covered) With A Sheet. 7) The Body Of A Deceased Person Should Be Marked With An Identifier By A Nurse, Ward Nurse, Midwife From The Ward Where The Death Occurred, In Accordance With Applicable Regulations. 8) The Contractor Ensures That The Body Is Collected Each Time By Two Employees Of An Authorized Company Transporting The Body To The Cold Store/Dissecting Room (In Accordance With The Current List Of Names Of Persons Authorized To Perform The Contract Regarding The Collection And Transport Of Bodies).9) The Contractor Is Obliged To Collect The Body From The Hospital Departments Within The Specified Period.
