The Subject Of The Order Is The Provision Of Services In The Field Of Organization And Supervision Of The Comprehensive Implementation Of A Non-Commercial Clinical Trial, As Well As The Provision, Maintenance, Configuration And Provision Of Technical Support Of The System For Electronic Management Of Non-Commercial Clinical Trials (Hereinafter Referred To As The System), As Part Of The Project Entitled Prospective, Multicenter, Randomized, Three-Arm, Phase Ib/Iii, Comparative Study Aimed At Optimizing And Personalizing Immunological Therapy For Relapsed Chronic Lymphocytic Leukemia Patients (Optic) (Hereinafter: Project). 2. Based On Article. 18(4) Of The Public Procurement Law, The Ordering Party Introduces An Obligation To Keep The Information Contained In Annex No. 1A To The Description Of The Subject Of The Order Confidential. “Detailed Description Of The Clinical Trial.” A) Confidential Information Included In Annex No. 1A To The Description Of The Subject Of The Order Entitled The Detailed Description Of The Clinical Trial, Available On The Purchasing Platform In The Appendices Tab, Has Been Password Protected. B) In The Questions/Information Tab, The Contractor Submits An Application For Access To The Password For Annex No. 1A To The Description Of The Subject Of The Order Entitled Detailed Description Of The Clinical Trial Along With A Statement According To Annex No. 13 To The Swz - Contractors Declaration On Maintaining The Confidentiality Of The Provided Data. C) If The Declaration (According To Annex No. 13 To The Swz) Is Not Submitted Along With The Request For The Password To Confidential Information, The Ordering Party Reserves That The Request Will Not Be Considered. D) The Declaration, In Accordance With Annex No. 13 To The Swz, Is Submitted By The Contractor In An Electronic Form Signed By A Person Authorized To Act On Behalf Of The Contractor With A Qualified Electronic Signature. E) The Declaration According To Annex No. 13 Must Include Data, Including E-Mail Address And/Or Mobile Phone Number, Regarding The Person Signing The Declaration According To Annex No. 13, I.E. The Person Authorized To Act On Behalf Of The Contractor He Represents. F) Based On The Data Contained In The Declaration, The Ordering Party Will Verify The Correctness Of Its Submission. In Case Of Doubt, The Ordering Party Reserves The Right To Request The Contractor To Supplement Or Correct The Declaration.
