The Subject Of The Order Is The Provision Of Services In The Field Of Conducting A Non-Commercial Clinical Trial, As Well As Providing, Maintaining, Configuring And Providing Technical Support To The System For Electronic Management Of Non-Commercial Clinical Trials (Hereinafter Referred To As The System), As Part Of The Project Entitled Randomized Phase Iii, Partially Blinded Study Comparing The Effectiveness And Safety Of Clobetasol, Mometasone And Tacrolimus In The Treatment Of Vulvar Lichen Sclerosus In The Pediatric Population (Hereinafter: Project). 2. A Detailed Description Of The Subject Of The Order Is Included In Annex No. 1 To The Swz Description Of The Subject Of The Order (Also Called Opz), Which Consists Of: 2.1. B) In The Questions/Information Tab, The Contractor Submits An Application For Access To The Password For Annex No. 1A To The Description Of The Subject Of The Order Entitled Detailed Description Of The Clinical Trial Together With A Statement According To Annex No. 13 To The Swz - Contractors Declaration On Maintaining The Confidentiality Of The Provided Data. C) If The Declaration (According To Annex No. 13 To The Swz) Is Not Submitted Along With The Request For The Password To Confidential Information, The Ordering Party Reserves That The Request Will Not Be Considered. D) The Declaration, In Accordance With Annex No. 13 To The Swz, Is Submitted By The Contractor In An Electronic Form With A Qualified Electronic Signature By A Person Authorized To Act On Behalf Of The Contractor. E) The Declaration According To Annex No. 13 Must Include Data, Including E-Mail Address And/Or Mobile Phone Number, Regarding The Person Signing The Declaration According To Annex No. 13, I.E. The Person Authorized To Act On Behalf Of The Contractor He Represents. F) Based On The Data Contained In The Declaration, The Ordering Party Will Verify The Correctness Of Its Submission. In Case Of Doubt, The Ordering Party Reserves The Right To Request The Contractor To Supplement Or Correct The Declaration. G) The Ordering Party Will Provide The Password To The E-Mail Address And/Or Mobile Phone Number Of The Person Authorized To Act On Behalf Of The Contractor Indicated In The Declaration.
