The Subject Of The Order Is The Implementation Of Tasks In The Field Of Government Administration Resulting From The Comprehensive Support Program For Families Za Życiem, Constituting An Annex To Resolution No. 189 Of The Council Of Ministers Of December 27, 2021 On The Comprehensive Support Program For Families Za Życiem (M.P. 2022, Item 64) Hereinafter Referred To As The Programme In The Scope Of The Implementation Of The Tasks Of The Leading Coordination, Rehabilitation And Care Center In The Lublin Poviat, Specified In Art. 90V Section 4 Of The Act Of September 7, 1991 On The Education System (Journal Of Laws Of 2021, Item 1915, As Amended) Resulting From Task 2.4, Priority Ii Of The Program: Multi-Faceted And Comprehensive Assistance For A Disabled Child In The Period From About One Year Of Age Until Starting School And His Family For The Psychological And Pedagogical Counseling Center In Bełżyce, Ul. Fabryczna 2B, 24 – 200 Bełżyce By Specialists In The Field Of Psychology. The Program Is Addressed To Children From The Age Of 0 To The Time Of Starting School Who Have Been Diagnosed With A Disability Or A Risk Of Disability. Services May Be Provided At The Psychological And Pedagogical Counseling Center In Bełżyce, Ul. Fabryczna 2B Or In Another Place Located In The Lublin Poviat Or The City Of Lublin, Established In Accordance With The Provisions Of The Regulation Of The Ministry Of National Education Of August 24, 2017 On The Organization Of Early Support For The Development Of Children (Journal Of Laws Of 2017, Item 1635). Classes Will Be Held Will Be In Accordance With The Number Of Hours Allocated To The Child, Determined By The Diagnosing Team Selected By The Director Of The Psychological And Pedagogical Counseling Center In Bełżyce Ul. Fabryczna 2B, From Among Specialists Qualified To Implement The Program, According To The Established Schedule Of Classes. The Deadline For The Service Is December 15, 2024. Classes Are Conducted Individually With The Child And His Or Her Family. As Part Of Psychological Classes, In Order To Develop Social And Communication Competences Necessary For The Child To Function In Social Life, Classes May Be Conducted In A Group, With The Participation Of Childrens Families Or Other Children Receiving Support. The Number Of Children In A Group Cannot Exceed 3 (This Applies To Tus Social Skills Training Classes). Group Classes Are Organized Depending On The Childs Developmental Needs In Accordance With The Recommendations Of The Consulting Team. The Ordering Party Does Not Reserve The Obligation For The Contractor To Personally Perform The Works Covered By The Subject Of The Order. The Contractor May Entrust The Implementation Of Part Of The Order To A Subcontractor. The Term Hour Means 60 Minutes. The Ordering Party Emphasizes That The Term Hours Of Provision Of Services Should Be Understood Only As The Actual Time Of Providing Services To Or For The Child. The Costs Resulting From The Scope Of Implementation Of The Subject Of The Order Are Covered By The Contractor Within The Remuneration Specified In The Offer, Constituting The Maximum Obligation Of The Ordering Party. The Working Time Of Individual Specialists Will Be Determined In Accordance With The Psychophysical And Developmental Needs Of Children And Their Families. The Ordering Party Allows For The Possibility Of Reducing The Number Of Hours Allocated To Contractors By No Less Than 50% Of The Basic Number Of Hours Specified In The Contract. Failure Of The Ordering Party To Allocate The Full Number Of Hours Specified In The Contract To A Given Contractor Will Not Result In The Contractors Claim For Performance And Payment Of Remuneration For The Remaining Number Of Uncompleted Hours. The Order Covers The Implementation Of The Subject Of The Order In The Amount Of 100 Hours. Scope Of Responsibilities Of The Psychologist: • Conducting A Preliminary Diagnosis, • Participation In The Meeting Of The Consultation Team Referred To In The Regulation Of The Ministry Of National Education Of August 24, 2017. On Organizing Early Support For Childrens Development (Journal Of Laws Of 2017, Item 1635), • Developing And Implementing An Individual Development Support Program With The Child And His Family, In The Part Intended For A Psychologist, • Assessing The Childs Progress And Difficulties In Functioning Once A Year Within The Scope Of Their Classes, • Analyzing The Effectiveness Of The Assistance Provided Under The Program To The Child And His Or Her Family, Introducing Changes To The Program According To The Childs Current Needs, • Planning Further Development Support Activities, • Organizing Meetings And Consultations For The Childs Parents And Legal Guardians The Child, The Childs Actual Guardians (Their Number May Not Exceed 15% Of The Total Hours Spent With A Given Child), • Maintaining The Required Documentation (Recommendations Of The Consulting Team, A Journal Of Specialized Activities, The Childs Individual File), • Preparing And Submitting A Monthly And Annual Report Within The Required Deadline From The Implementation Of Classes. In Addition: • Determining The Childs Level Of Functioning On The Basis Of The Diagnosis Included In The Opinion On The Need For Early Support For The Childs Development, And In The Absence Of The Above-Mentioned Opinion, On The Basis Of An Individual, Preliminary Diagnosis In Accordance With The Qualifications, Based On Information About The Detection Of A Risk Of Disability Or Disability , Direction, Scope And Schedule Of Proposed Activities In The Field Of Early Assistance And Support For The Childs Family, Or Other Specialized Activities Taking Into Account The Development Of The Childs Activity And Participation In Social Life And The Elimination Of Barriers And Restrictions In The Environment That Hinder Its Functioning; • Cooperation With The Childs Family, In Particular By: Providing Assistance In Shaping Desirable Attitudes And Behaviors In Contacts  With The Child: Strengthening The Emotional Bond Between Parents And The Child, Recognizing The Childs Behavior And Consolidating Appropriate Reactions To These Behaviors, Providing Instructions And Advice, And Conducting Consultations On Working With The Child, • In The Absence Of An Opinion On The Need For Early Support For The Childs Development, Cooperation With The Appropriate Psychological And Pedagogical Counseling Center And Providing Assistance To Parents In Order To Conduct Tests And Establish A Specialized Diagnosis Of The Childs Level Of Functioning; • Developing And Implementing A Weekly Schedule With The Child And His Family; • Carrying Out, Within The Scope Of His Qualifications, Other Tasks Of The Coordination, Rehabilitation And Care Center Resulting From The Needs Of The Child And His Family, Including In Particular:  Identifying Other Tasks Appropriate For The Child And His Family Forms Of Specialized Assistance,  Establishing Cooperation With A Kindergarten, Another Form Of Pre-School Education Attended By The Child Or Other Entities Where The Child Is Subject To Therapeutic Interventions In Order To Ensure Coherence Of All Activities Supporting The Childs Development;  Establishing Cooperation With A Medical Entity In Order To Diagnose The Childs Needs Resulting From His Or Her Disability, Providing Him With Medical And Rehabilitation Support And Recommended Medical Products, As Well As Advice And Consultations On Supporting The Childs Development;  Establishing Cooperation With Organizational Units Of The Family Support And Foster Care System; • Analyzing The Effectiveness Of Assistance Provided To The Child And His Family, Making Changes To The Program According To The Needs Of The Child And His Family, And Planning Further Actions. • Maintaining The Following Documentation: A) An Individual Program That Specifies In Particular: - How To Implement Development Goals Aimed At Improving The Childs Functioning, Strengthening His Or Her Participation In Social Life And Preparing For School, Eliminating Barriers And Restrictions In The Environment That Hinder The Childs Functioning, In Including His Activity And Participation In Social Life; - Support For The Childs Family In Implementing The Program; - Method Of Assessing The Childs Progress; B) The Childs Individual File, Which Contains: - Parents Application For The For Life Program; - Interview Questionnaire With Parents; - Recommendations Of The Consultation Team; - Medical Documentation; - Decisions On Special Education, Opinion On The Need For Early Development Support (If The Child Has Such); - Psychological, Pedagogical And Speech Therapy Documentation; - Other Relevant Documents; C) A Register Of Individual Classes With The Signature Of Parents Confirming Their Implementation; D) Monthly And Annual Or, In The Event Of Early Completion Of Classes, A Final Report On The Classes Conducted By December 15 For The Year, Including In Particular: - Description Of The Task, Detailing The Activities Carried Out; - Material, Numerical And Qualitative Effects Of Implemented Activities.
