The Subject Of The Order Includes The Provision Of Services Providing 24-Hour Shelter In A Shelter For Homeless People Referred By Mgops In Opoczno. Detailed Description Below: 1. The Ordering Party Anticipates That The Projected Average Number Of Homeless People Covered By The Service Will Be Approximately 16 People (Women And Men).2. The Ordering Party Reserves The Right To Reduce/Increase The Number Of Homeless People Covered By The Service Indicated In Section 1 According To The Actual Demand, Depending On The Actual Needs Of The Ordering Party (Respectively The Number Of Administrative Decisions Granting The Benefit Actually Issued), Of Which The Contractor Will Be Informed In The Manner Indicated In § 1 Section 13 In The Draft Provisions Of The Contract. 3. The Ordering Party Indicates The Minimum Value Of The Contractors Remuneration For The Performance Of The Subject Of The Order In The Amount Of 35% Of The Remuneration Referred To In § 3 Section 1 In The Draft Provisions Of The Contract.4. Each Time A Homeless Person Is Placed In A Shelter, It Is Based On An Administrative Decision Granting Shelter.5. The Ordering Party Reserves The Right To Supervise And Control The Implementation Of The Subject Of The Contract And Other Obligations Arising From The Contract.6. The Subject Of The Order Is To Be Implemented In Accordance With The Provisions Of The Act Of March 12, 2004. On Social Assistance (Consolidated Text: Journal Of Laws Of 2023, Item 901, As Amended) And The Regulation Of April 27, 2018 On Minimum Standards For Night Shelters, Shelters For Homeless People, Shelters For Homeless People With Care Services And Heating Rooms (Journal Of Laws Of 2018, Item 896).7. The Contractor Is Obliged To Cooperate With The Ordering Party In The Implementation Of Individual Programs For Ending Homelessness And Concluded Social Contracts.8. The Contractor Provides Assistance To Shelter Residents In Life- Or Health-Threatening Situations By Enabling First Contact With A Doctor Or Health Care Facilities.9. The Contractor Ensures Ongoing Contact Between The Shelters Social Worker And The Mgops Employee In Opoczno From 7.30 A.M. To 3.30 P.M., Monday To Friday, And On Tuesdays From 7.30 To 16.30. The Current Contact Referred To In This Point Must Enable Immediate Action To Be Taken To Check, Verify And Provide Information About The Current Situation Of A Given Person Staying In The Shelter.
