The Scope Of The Order Includes:1. Developing The Rules And Operating Scheme Of The Lottery, In Consultation With The Ordering Party, Within 10 Business Days From The Date Of Conclusion Of The Contract;2. Preparation Of The Lottery Regulations, In Consultation With The Ordering Party, Within 10 Business Days From The Date Of Conclusion Of The Contract;3. Establishing Security For The Payment Of Prizes Required By The Gambling Act By Obtaining Bank Guarantees For The Payment Of Prizes For The Purposes Of The Lottery, For An Amount Equivalent To The Prizes To Be Awarded To The Lottery Winners In Accordance With The Lottery Regulations, And The Contractor Is Obliged To Apply For A Bank Guarantee No Later Than 5 Business Days From The End Of The Previous Stage - I.E. From The Date Of Development Of The Rules And Operating Scheme Of The Lottery And The Lottery Regulations;4. Obtaining Permission To Conduct The Lottery, Including: Submitting An Application To The Territorially Competent Director Of The Tax Administration Chamber, No Later Than 2 Business Days From The Date Of Receipt Of The Bank Guarantee, In Order To Obtain A Decision Enabling The Organization Of The Lottery. In The Submitted Application, The Contractor Will Be Indicated As The Entity Organizing The Lottery - The Organizer Of The Lottery. The Contractor Is Obliged To Send The Ordering Party Confirmation Of The Date Of Receipt Of The Application To The Director Of The Tax Administration Chamber;5. Immediately Providing The Ordering Party With A Copy Of The Obtained Decision Authorizing The Organization Of The Promotional Lottery And The Bank Decision;6. Graphic Preparation And Printing Of 4,000 Copies Of Lottery Entry Forms No Later Than 3 Business Days Before The Start Of The Lottery;7. Providing And Delivering To The Places Indicated By The Ordering Party, Within 3 Business Days Before The Start Date Of The Lottery, 8 (In Words: Eight) Urns For Application Forms, And The Urns Should Be Marked With The Name Of The Lottery, Sealed And Secured To Maintain The Security Of Personal Data Contained On Submitted To The Above-Mentioned Ballot Boxes. Lottery Entry Forms;8. Collection From The Ordering Party And Delivery To The Places Indicated By The Ordering Party, No Later Than 3 Business Days Before The Start Date Of The Lottery, Of 6 (In Words: Six) Premium Roll-Ups And 2 (In Words: Two) Double-Sided Textile Walls With Information About The Lottery, For Its Purposes Promotion;9. Collection Of Ballot Boxes Containing Entry Forms From The Places Indicated By The Ordering Party After The Deadline For Entries To The Lottery;10. Preparation And Launch No Later Than 3 Business Days Before The Start Date Of The Lottery And Maintaining An Online Lottery Application Form On The Contractors Servers In The Period From The Start Date Of The Lottery No Later Than 3 Months From The End Date Of The Lottery. After Launching The Form, The Contractor Will Provide The Ordering Party With The Address Of The Website Where The Online Application Form Is Located In Order For The Ordering Party To Redirect From The Ordering Partys Website. After The Drawing Ends, On The Same Day, The Contractor Will Post A List Of Winners In Place Of The Form;11. Appointing A Supervision Committee To Control The Correctness Of The Draws Performed, As Well As To Conduct Complaint Proceedings In Accordance With The Currently Applicable Legal Provisions Regarding Possible Claims Submitted By Lottery Participants;12. Supervision And Coordination Of The Lottery, Drawing And Fulfillment Of All Obligations Arising From The Lottery Regulations, With The Prize Drawing Taking Place In Lublin At A Place Provided By The Ordering Party;13. Systematic And Correct Preparation Of Documentation Required For The Operation Of The Lottery, And In Particular For The Operation Of The Drawing;14. Purchase Of The Main Prize For The Lottery;15. Providing An Additional Cash Prize For The Main Prize Winner In The Amount Of: 11.1111% Of The Value Of The Main Prize, Which Will Not Be Handed Over To The Main Prize Winner, But Will Be Withheld By The Contractor And Intended To Cover The Due Personal Income Tax, For Which The Participant, By Taking Participation In The Lottery, Must Consent;16. Payment Of Additional Prizes To Winners - Cash Prizes In The Lottery;17. Delivering The Main Prize To The Place Indicated By The Ordering Party And Handing It Over To The Main Prize Winner;18. Preparing Documentation For The Issuance Of Prizes And Settling The Lottery In Accordance With The Requirements Of Applicable Regulations, In Particular By Paying Fees To The Tax Administration Chamber;19. Obtaining The Consent Of The Lottery Winners To The Processing Of Personal Data And Image For The Ordering Partys Promotional Needs;20. Collecting And Processing Personal Data Of Lottery Participants In Accordance With Applicable Law, Only To The Extent Necessary To Perform The Obligations Of The Lottery Organizer;21. Ensuring The Participation Of The Contractors Representative At The Main Prize Award Ceremony, Which Will Take Place In Lublin On A Day Agreed In A Working Manner With The Ordering Party;22. Ensuring All Necessary Logistics Related To The Implementation Of The Lottery Resulting From Its Regulations, Including: Purchase, Transport And Delivery Of The Prize, Transport Of Advertising Media, Supervision Of Ballot Boxes;23. Conducting Complaint Proceedings; 24. Preparation And Submission To The Customs And Tax Office Of A Report After The End Of The Promotional Lottery And The Performance Of All Other Obligations Of The Lottery Organizer;25. Preparing A Report On The Execution Of The Subject Of The Order, Including Documentation Of The Execution Of All Elements Of The Subject Of The Order, Including Photographic Documentation.
