4.1. The Subject Of The Contract Is The Provision Of Medical Services In The Field Of Preventive Medical Examinations And Preventive Health Care For Job Candidates And Employees Employed In The District Prosecutors Office In Warsaw And Its Subordinate District Prosecutors Offices. 4.2. The Subject Of The Contract Includes In Particular:4.2.1. Performing The Following Health Services By The Contractor: A) Initial Examinations Of Candidates For Work In The Public Prosecutors Office, B) Periodic Examinations Of Employees, C) Control Examinations Of Employees, Provided For In The Labor Code By A Physician Authorized To Perform Them. 4.2.2. Performing Examinations In The Field Of Preventive Health Care Referred To In 6 Of The Regulation Of The Minister Of Health And Social Welfare Of 30 May 1996 On Medical Examinations Of Employees, The Scope Of Preventive Health Care For Employees And Medical Certificates Issued For The Purposes Provided For In The Labor Code (I.E. Journal Of Laws Of 2023, Item 607).4.2.3. The Ordering Party Estimates That During The Performance Of The Contract, It Will Order: A) Preliminary/Periodic Tests In The Amount Of: 646, B) Control Tests In The Amount Of: 90, C) Specialist Consultations Pursuant To 2 Sec. 2 Of The Regulation Of The Minister Of Health And Social Welfare Of 30 May 1996 On Medical Examinations Of Employees, The Scope Of Preventive Health Care For Employees And Medical Certificates Issued For The Purposes Provided For In The Labor Code, In The Amount: 15.
