Information About The Project The Subject Of The Contract Is The Provision Of The Contract Engineer (Ik) Service Under The Project Entitled Adaptation Of It Systems And Infrastructure To The Growing Needs In The Area Of Documentation Of Medical Events, Integration With Central Systems, Obligations In The Field Of Edm Production And The Need To Maintain The Security Of Patient Data. Is A Public Health Care Institution, Whose Founding Body Is The Capital City Of Warsaw, Providing Health Care Services Since 1992. The Mission Of The Department Is To Provide High-Quality Medical Services And Promote Health. The Team Consists Of 13 Medical Clinics Located In Two Districts Of The Capital City Of Warsaw: Wola And r dmie cie. These Outpatient Clinics Provide Health Care Services In The Following Areas: Primary Health Care, Specialist Outpatient Care, Medical Rehabilitation, Dental Treatment, Occupational Medicine, Health Prevention And Promotion, Psychiatry And Alcohol Addiction Treatment, Imaging Diagnostics Laboratory Diagnostics. As Of Today, The Ordering Party Uses Software Supporting Its Substantive Work (Including The Creation And Maintenance Of Electronic Medical Records), For Which It Has A Non-Exclusive, Unlimited In Time And Unlimited Territorial License. The Ordering Party Is Considering The Purchase (Licensing) Of New Software Due To The Need To Adapt Its It Systems And It Infrastructure To The Growing Needs In The Area Of Documentation Of Medical Events, Integration With Central Systems, Obligations In The Field Of Emd Production And The Need To Maintain Patient Data Security (Project) . The Ordering Party Intends To Systematically Increase The Possibilities Of: Handling Patients Cases Without The Need To Be Physically Present At The Clinic, The Best Protection Of The Medical Documentation Produced, Automating The Exchange Of Information With Cooperating Entities Providing Services For The Team And The Clinics Included In It, Including In Particular Automating The Integration With Cooperating Laboratories. The Scope Of Works Of The Engineer Has Been Divided Into Two Parts: 1) Basic - Covering The Assessment Of The Employers Existing It Resources, Its Needs In This Area And The Presentation Of Recommendations Regarding Possible Changes For The Future (Part Ii Section 6, Points 1-5 Of The Opz ),2) Optional - In The Case Of Acceptance By The Ordering Party Of The Contractors Recommendation Regarding The Public Procurement Model (Part Ii Section 6 Point 5 Of The Opz) And Having Adequate Financial Resources To Cover The Costs Of Purchasing New Software Along With The Necessary It Infrastructure - Including: Development Of Documentation For The Purposes Of Public Procurement Under The Project, Along With Providing Support In The Contracting Process (Participation In The Work Of The Committee, Answers To Questions) - Part Ii Sec. 6 Point 6 And Points 7-8 (Subject To The Following Reservation). At The Same Time, The Contractor Is Obliged To Support The Employer, Including Providing Information Necessary To Monitor The Project, Throughout The Term Of The Contract (Up To A Maximum Of 12 Months, In The Case Of Exercising The Option , And In The Case Of Not Exercising The Option Right, Until The Execution Of The Basic Part Of The Order) - Part Ii Sec. 6 Points 7-8 Opz.Ii. General Provisions1. Engr. Will Manage The Project In Consultation With The Employer.2. Engr. Is Obliged To Perform The Subject Of The Order With Due Diligence, On Time, In Accordance With The Applicable Provisions Of National And Community Law And The Decision On The Implementation Of The Project.3. Engr. Is Obliged To Perform The Subject Of The Contract In Accordance With The Guidelines, Instructions, Manuals And Other Documents Applicable To The Employer. 4. Engr. Is Obliged To Support The Employer In All Areas Related To The Implementation Of The Project And To Ensure A Consistent Work Methodology So That, As A Result Of The Performance Of Tasks By The Engr., Effective Project Management, More Efficient Response To Changes And Threats, Efficient Flow Of Information Between The Employer And The Contractor/ Contractors, And Coordination Of Activities.5. Is Obliged To Provide Services At Its Headquarters, The Employers And Project Implementation Sites, In Particular To Ensure The Presence Of Persons Intended For The Implementation Of The Contract In Response To The Request Submitted By The Employer.6. Scope Of Works Of The Engr.: 1) Analysis Of It Resources Owned By The Employer In The Following Facilities: A) Independent Public Health Care Center Warszawa Wola- r dmie cieb) Clinics Of The Plant Chmielna 14Cio ka 11Dragon w 6Aelekcyjna 54Grzybowska 34Leszno 17Maria ska 1Miedziana 2Nowolipie 31Obozowa 63/65P ocka 49Szlenkier w 10 ytnia 39, Including In Particular:- Owned It Systems, Scope Of Data, - Owned Network And Server Infrastructure, - Licenses For It, Domain And Standard Systems, With An Indication Of Whether They Allow The Performance Of Obligations Under The Law, - Service / Maintenance Contracts, With An Indication Of Their Scope And Deadlines -Assessment Of Systems And Procedures Used To Secure Patient Data, Based On Audits Previously Carried Out At The Request Of The Ordering Party, -Agreements With Entities Providing Services To The Team And Its Outpatient Clinics, Under Which Patient Data Is Exchanged, Including In Particular External Laboratories Within 45 Calendar Days From The Date Of Conclusion Of The Agreement; 2) Analysis Of The Ordering Partys It Needs: A) In The Area Of It Systems Supporting The Provision Of Clinic Services, E-Services As Well As Integration With Central Systems And Systems Of Cooperating Entities, B) In The Area Of The It Infrastructure Necessary For The Efficient Operation Of The Systems And To Ensure The Security Of The Systems And Patient Data Contained Therein, Within 45 Calendar Days From The Date Of Conclusion Of The Agreement, 3) Conducting A Market Analysis Of The Available It Systems Referred To In Point 2 Above, And Presentation To The Ordering Party Of Selected (At Least Three) Such It Systems4) Development Of Recommendations On How To Meet The Needs Defined As A Result Of The Analysis, With Particular Emphasis On The Recommendation Regarding The Purchase Model Of Outpatient Clinic Systems And Their Common Platform Or One Common System For All Facilities - Within 45 Calendar Days From The Date Of Conclusion Of The Agreement, 5) Preparation Of An Estimated Valuation Of Necessary Investments - Within 45 Days From The Date Of Conclusion Of The Agreement, 6) Preparation Of Recommendations Regarding The Public Procurement Model, Including The Number And Scope Of Individual Contracts, Together With Appropriate Justification, The Ordering Party Limits The Maximum Number Of Potential Orders To 15, Within 45 Days From The Date Of Conclusion Of The Agreement, 7) Development Of Documentation For The Purposes Of Public Procurement Under The Project, Along With Providing Support In The Contracting Process (Participation In The Work Of The Committee, Answers To Questions) - In Within 60 Calendar Days From The Date Of Receipt And Acceptance By The Ordering Party Of The Documentation Referred To In Points 1-5, 8) Support Of The Ordering Party In The Implementation Of The Project During The Implementation Of The Agreement - During The Term Of The Agreement - Within 12 Months From The Date Of Conclusion Of The Agreement,9 ) Providing Information Necessary To Monitor The Project - During The Term Of The Agreement - Within 12 Months From The Date Of Conclusion Of The Agreement - With The Proviso That The Scope Of Work Indicated In Points 6-8 Above Is Covered By The Option Right (With The Reservation Indicated In Part I In Fine The Ordering Party Will Commission Its Implementation If The Following Conditions Are Met Jointly: A) The Ordering Party Accepts The Recommendation Regarding The Public Procurement Model Referred To In Point 5 Above, B) The Ordering Party Has Appropriate Financial Resources To Cover The Costs Of These Orders Under The Project And C) Make A Decision To Prepare A Tender Procedure Or Procedures In Order To Implement The Project. Iii. Ci Obligations In The Area Of Risk Management Mik Is Obliged To Manage The Risk In The Implementation Of The Project: 1) Identifying, Analyzing, Monitoring And Reporting Risks And Assessing Their Impact On The Implementation Of The Project, 2) Developing And Updating The Risk Register, Which Is An Element Of The Monthly Reports, 3) Developing A Plan Preventive Actions, 4) Development Of Proposals For Corrective Actions, 5) Implementation Of Corrective Actions After The Employers Approval.
